Office of the City Clerk

The City Clerk’s Office is one of the main points of contact for many city residents and businesses. The office handles many of the information requests received from the public, in addition to processing license and permit applications. The city clerk also keeps records for the city and Common Council, including meeting minutes and updates to the city’s code of laws, when necessary.

Application - Birth Record

Complete this form and return to the clerk’s office to receive a copy of your birth certificate.

Application - Death Record

Complete this form and return to the clerk’s office to receive a copy of a death certificate. 

Application - Disability Parking Permit

The forms here are provided by the New York State Department of Motor Vehicles to apply for a disability parking permit.

This form can be used to obtain records for genealogical research purposes.

Complete this form and return to the clerk’s office to receive a copy of a filed marriage certificate.

Application - Public Access Records (FOIL)

This FOIL (Freedom of Information Law) application can be submitted to the clerk’s office to access publicly held records.

Board of Ethics Official Review Form

This form can be submitted to the clerk’s office to request a review by the city’s Board of Ethics.

Peddlers Pemit

The City of Lockport requires that all peddlers operating within city limits apply for and carry a permit. No person shall engage in a business, occupation or activity in the public streets and places and from door to door within the city without first applying for and securing a license. 

Taxi Cab License

Any driver interested in serving as a taxi cab driver in the City of Lockport should complete this license application and submit it to the City Clerk’s office.
